Good morning Ken,
 
going by the series of emails that finally found its way to you, I hope you are the one to be checking with on this Licence Spill Contingency Plan.
 
I had this plan passed onto me from Phyllis yesterday (see email below), and looking back at the licence, under Part G, Item 1, the licensee was to submit an "addendum" or addition to the conditionally approved Spill Plan that was to address the six items listed.  These items were not addressed in the Plan that was submitted with the email below and remain outstanding.
 
Not sure why the Spill Report Form was listed under Part G, Item 1, as it was in the original submission (maybe it didn't print off or something!!), but the others are still valid and are required to be updated for the Plan prior to receiving final approval of the Plan.
 
Also, if submitting a complete Plan, please identify which items have been updated, as well as a revision date on the Plan.
