morning Erik. I've just returned from vacation and noticed that no action
has been taken on your request. I would like to get this one out the door
as quickly as possible but in order to do that we require the 3 page application
form and the $30.00 administration fee (in cheque form made payable to Receiver
General for Canada). Please forward the application form electronically so
