Good morning John.  This email is sent to notify you that the Nunavut Water Board has not received an annual report from Starfield since 2004.  Please use the attached form and submit reports for 2005 and 2006.  Thank you.  I also want to remind you that your current license expires on July 1, 2007.  If you plan on renewing the license, might I suggest that you submit same as soon as possible in order to obtain the new license prior to the expiry.
