Re: Renewal of Water
License NWB2JACO203
Dear Ms.
Phyllis,
I am geological consultant from MPH
Consulting Limited,
MPH had submitted the renewal form in April
2006. However, MPH had been informed that the application had not reached
the office (which probably may have forwarded to different
location).
Thus, MPH is forwarding renewal application
with this email. The files attached with this email
includes
-Spill Contingency Plan for renewal of
License NWB2JAC0203 and
-Abandonment and Reclamation Plan for Water
License NWB2JAC0203-to accompany 2006 renewal
application.
NWB Annual Report for Y2005, License
NWB2JAC0406/B2 had been already been forwarded by Dallas Davis on March 2006.
I hope this will prove satisfactory and
please feel free to contact us for further
information.
Thanking you in
advance.
