1.  Admin fee of $30.00 and Water Use Fee of $30.00, payable by cheque,
money order or visa.  If paying by cheque it must be made payable to
"Receiver General for Canada"
2.  Project summary in english and inuktitut
3.  Project Area Map
4.  Spill Contingency Plan
5.  Abandonment and Restoration Plan
